If the blender is too fast, it will break the emulsion. A medium steady
speed is best. That said, these days I make mine by hand with a big
wire whisk. Always drizzle the oil slowly.

My old Cuisinart has a pusher with a tiny hole in the bottom, into
which you can pour oil and have it dribble itself gradually into the
mixture.
