It seems to be a "drive access" problem.
In "all" cases, I have gotten rid of this sort of thing,
(if the drive was still working properly), by using
either the small IBM utility ZAP or WIPE.
(I even use it to start over on an XP install I did not "like"...)
It is from the "era" of your drive... (1996)
I would prefer you get it from the "Net" if you can...
