I'm also lactose intollerant, with similar symptoms.

Like you, my solution is to avoid all dairy products, and being severely
intollerant (perhaps having sensitized myself a bit when I was drinking
milk to settle my constantly upset stomach), I take a lactose pill with
every meal, whether I think there is milk/lactose in it or not.

This seems to help.

Lactose can sneak into surprising places: things like fried onions,
crispy/curly/seasoned french fries, fried chicken, etc.

Breaded fried things are especially bad, since many recipes call for
dipping the major ingredient into milk before breading it, to help the
breading stick, and sometimes lactose is added to the breading because
is browns nicely.

Also watch for lactose fillers in medications, and make sure your
pharmacist knows to watch for them.

When I get a dose, I eat 3 or 4 Nature's Valley Granola bars - or
anything else I can force myself to eat that's got a lot of fiber, to
push the offending materials through my system as fast as possible.  The
Gas-Ex type pills help relieve some of my gas-induced discomfort.

All things considered, once you realize the problem, lactose
intollerance is the easiest of the food allergies to live with.  Keep
the lactase pills handy, avoid the obvious stuff, and get used to Pizza
without cheese (or develop a liking for non-milk cheeses), and life
proceeds.
