i don't know anything about the Latin word for
"worm-shaped form", but the word appendix is
etymologically derived from the Latin verb
_appendere_, which means "to hang upon".
think about the way the "appendix" at the back
of a book is "hung upon" the rest of the text
of the book ... get it?
