Did anyone on this list start with a new clean or an old formatted drive,
and using the XP upgrade CD do a clean full install?  If so, how?

Joseph,

I was running W98SE and did a clean install of WinXP after using fdisk,
formatting the hard drive. During the WinXP installation I inserted the W98
disk when asked to check for previous Win OS, when verified, continued on to
install WinXP. No problems and a very smooth installation.
