There are many variations of the Paleo diet, such as ones that are more
plant/raw food oriented. You don't have to emphasize meat. I actually eat
meals where 3/4 of the plate consists of vegetables and salad. I like the
variety and fiber. Perhaps you can approach him on how everyone now knows
how healthy fruits and vegetables are for one's health - as opposed to
stored, lifeless food. It will be quite natural for him to fill up the rest
of the plate with meat, and since meat has more calories than vegetables,
your patient will be getting probably half his calories from meat without
him viewing it as a meat-based diet. I don't mean to sound manipulative, but
rather offer a different perspective.

And don't forget that success can be quite motivating. Offer some good
success stories.
