If a paleo person has had a terminal ileum resection--the
site where B12 is absorbed--or is deficient or lacking in
intrinsic factor secretion from gastric cells because of
some other health condition, it seems quite possible for
them to be B12 deficient. It just doesn't seem like it
would be all that common, as it is in veganism.
