You have set your Network properties to:
    Primary Network Logon = Client for Microsoft Network

Go to Control Panel and Click on Network, you will find the windows
to change this in the lower portion of the windows that appears.

Change it to:
    Primary Network Logon = Windows Logon

Now, reboot your system.  It will ask for your user name and password
one last time and then this will be gone.  Be sure not to enter a password,
just a user name.
