Based on your light test sounds like your CCD is ok. A bad
CCD unit yields a all black output. Any chance this unit was
powered with shipping retainers still in. This can cause the scan
head to be out of step with the fixed focal point of the lens. If
this is the case, you're looking at a difficult calibration procedure.

Depends on the resolution, but there should have been
calibrations to go by, unless it's factory set only. Otherwise,
try to center it the best you can.

If you still have the orginal literature for your scanner you can
write or fax them for a parts list. Also, they should accept similar
requests
from their website. A word of caution, any two parts could easily exceed
replacement costs of a new non-HP scanner.
